Completely fill out the form and sign your name as you intend the account to be set up.
Your application will not be processed without a signature or proof
of identity. Please mail your application
to HEA and a copy of photo identification (i.e. state-issued
identification card, driver's license, passport, etc.). **If you are a new member to HEA, please enclose a $5.00 membership fee and
$10.00 records charge. A service representative will contact you regarding the security
deposit required for the meter location. A security deposit may be waived with a letter of
credit from a prior utility or can be paid in person, by mail, credit card or a payment
arrangement may be requested.
