Just complete the “3rd Party Notification” section on the front of the application and we will mail a copy of the delinquent notice to the specified address.
1. The Med-Alert Program will provide HEA members enrolled in the program reasonable prior notice of planned power interruptions due to scheduled service maintenance allowing members time to secure back-up power for their medical equipment or to make alternate arrangements.
2. The Med-Alert Program will not provide HEA members enrolled in the program prior notification for power interruptions or failures due to natural causes or unforeseen system problems. In these situations, it is the member’s responsibility to have a power back-up system for their medical equipment as well as an action plan for alternate arrangements or for proceeding to the nearest medical facility.
3. Members enrolled in the Med-Alert Program are not exempt from service disconnection if regularly scheduled payments are not made, payment arrangements are broken, or failing to revalidate participation in the program.
